Removing the epithelium (as done with CXL) creates a painful recovery and significantly increases risk of infection, corneal haze, scarring, and corneal nerve trauma requiring 6 months to recover. The non-invasive Holcomb C3-R® allows patients to return to their normal routine the next day! Our studies and studies of other eye doctors have shown patient results are not significantly different with crosslinking with removal of epithelium vs. leaving epithelium undisturbed in its place. Risks of Holcomb C3-R®
The risk of the Holcomb C3-R® Crosslinking System is mild irritation for a couple of days. And most people NEVER even experience that. That’s it.
Risks of Epi-off CXL
The complication rate of “epi-off” (removal of epithelium) CXL crosslinking procedure was recently reported to be high at 11%. Complications of CXL include:
1) severe pain
2) corneal infection and ulceration
3) corneal haze
4) delayed epithelial healing
5) worse, more blurry vision
6) cornea nerve damage persists even 5 years after CXL
7) permanent corneal scarring
8) induced visual distortions
9) peripheral flap melting in patients who had previous LASIK and flap dislodgement
10) cornea swelling that never goes away
11) damage to iris causing change in eye color
12) extended light sensitivity
13) cornea perforation (hole was accidentally made through entire thickness of cornea)
The above CXL complications have NEVER occurred with the Holcomb C3-R® Crosslinking System.

2) Holcomb C3-R® Crosslinking System covers the greatest corneal area for crosslinking – it covers 157% more surface area of the cornea compared to other techniques such as CXL. This allows for a more complete area of treatment which is important since Keratoconus extends into the periphery of your cornea . We make sure we treat your central cornea areas as well as the far periphery of your corneas with the Holcomb C3-R® Crosslinking System.
This is the reason that 99.3% of people are stabilized with just a single treatment with Holcomb C3-R® versus CXL stabilizing ONLY 71.6% of cases.
3) Some doctors are now “trying” epi-on crosslinking. Do not be fooled. 1) They are using solutions that do NOT have the long track of the proprietary solution used in Holcomb C3-R® and 2) Those epi-on patients are being “under-crosslinked” because the device these doctors use has ONLY a LOW energy setting. Epi-on crosslinking MUST be done with higher energy levels to produce more crosslinking. Dr. Brian’s Holcomb C3-R®, invented in 2003, does that and THAT is part of the reason the success rate of Holcomb C3-R® is 99.3%.

Patients with very thin corneas cannot be treated with CXL. Even if you have an extremely thin corneas, the good news is you can still be treated with Holcomb C3-R® to strengthen your corneas and help prevent you from needing a cornea transplant.
CXL is a SURGICAL procedure. In contrast, Holcomb C3-R® is NON-surgical hence NO COMPLICATIONS.
Holcomb C3-R® Crosslinking System is a routine part of our practice and has been so for over 10 years. Since Holcomb C3-R® is NOT in a study – our patients are not required to have the multiple “back and forth” office visits which can be quite time-consuming and disruptive to one’s school or work schedule.

Some patients take a “wait and see” approach with Keratoconus. The risk of waiting is that as the cornea continues to bulge out and get worse, the back layer of the cornea can rip open – like someone wearing way too tight pants who sits down and the pants split wide open down the middle seam. When this happens to the progressively bulging cornea, the internal eye liquid rushes in and swells up the cornea the same way a dry sponge soaks up water and becomes very thick.
These photos below show a cornea swollen up like a soppy, wet sponge after it split open. If this happens to you, it will take 3-6 months of non-usable vision until this fluid goes away. When it eventually does, the cornea will have a lot of new scarring and vision will be permanently worse compared to before it ripped open. The good news is that this devastating damage can easily be prevented with the 30-minute, non-invasive, 1-day recovery Holcomb C3-R® – it will give you important peace-of-mind.

2010 Vision Awards
In 2010 Dr. Brian was honored along with his patient, Olympic Gold Medalist Steven Holcomb. Both are credited to increasing awareness in 2010 for modern Keratoconus treatments. The Jules Stein Living Tribute Award was presented to Dr. Brian for pioneering alternative treatments to the painful cornea transplant for Keratoconus patients. Steven Holcomb was presented with the Athlete of Vision Award for raising awareness that Keratoconus can be treated and vision restored. Steven had retired from bobsledding in 2007 when Keratoconus began to drastically affect his ability to safely drive his bobsled at mock speeds. Steven’s coaches knew Steven was the best hope for Olympic Gold so they researched the Keratoconus options and discovered Dr. Brian.After being treated by Dr. Brian in 2007, Steven went on to win World Cup events in 4-man bobsled and then triumphed at the 2010 Winter Olympics securing the Gold Medal in the 4-man bobsled event, the first for the USA in 62 years. At the Vision Awards several celebrities were presenters, including Will.i.am of the widely popular music group the Black Eyed Peas. Other honorees that evening included Buzz Aldrin (astronaut & 2nd person to walk on the moon), Lilly Tomlin (comedienne, best know for her characters Ernestine and Edith Ann on the show Laugh-In) and Steve Jobs (former Apple CEO).
